Neuromuscular Blocking Actions of Mivacurium in Pediatric Patients




Abstract
Mivacurium(Mivacron(r), Burrows Wellcome Co., North Chicago, USA)is a new nondepolarizing neuromuscular blocking agent which is known for rapid onset and short duration of action. Mivacurium is bis-benzylisoquinolium diester and like
succinylcholine
rapidly hydrolized by plasma cholinesterase1.2). It's plasma elimination half life(T1/2) is 18.4 minutes and volume of distribution(Vd) is 112ml/kg3).
12 pediatric patients (ASA physical class 1) were anesthetized with thiopental sodium and N2O with either Ethrane or halothane for the study of mivacurium as a neuromuscular blocker.
Mivacurium 0.25 mg/kg was injected intravenously as a single bolus at induction.
T1 and T4 for adductor pollicis of thumb were recorded with Datex ABM Monitor(Datex instrumentarium Corp. Helsinki, Finland) stimulating TOF at 2Hz with ulnar nerve at wrist every 20 seconds.
Blood pressure and heart rate were monitored and recorded using noninvasive instruments every 1 minutes.
Intubations were done after T1 ration had reached below 10% and all were under good to excellent conditions. The mean time from injection of mivacurium to intubation was 3.6¡¾0.8minutes.
Mean T0 is 1.9¡¾0.7 minutes.
Recovery index(T25-75) was 5.7¡¾2.6 minutes.
There is no statistically significant effect on blood pressure and heart rate before and after mivacurium injection.
Conclusion : It was concluded that mivacurium has the rapid onset and short duration of the neuromuscular blocking actions without specific cardiovascular effect in pediatric patients.
